A preventative approach to health care results in better patient care and better patient outcomes. This, in turn, leads to lower costs to the healthcare system. Patients who receive education on their overall health need to become empowered to take part in their own health and disease prevention.

As part of MPAC Healthcare's vision of transforming how healthcare is delivered to the aging population, we are adding a preventative care program to the work that our clinicians are already doing at our partner facilities.

In addition to addressing acute and chronic medical needs on a daily basis, the MPAC Healthcare in-house Nurse Practitioner will begin screening patients eligible for an Annual Wellness Visit and an Advanced Care Planning Session. These programs are CMS initiatives and are an effort to focus on education and disease prevention and management in the aging population.

Our Preventative Care Program does not compete with or replace the facilities' palliative/hospice provider. To the contrary, we work in collaboration with them to provide the palliative care that can be cost prohibitive for them to provide on their own.

These services are covered by Medicare Part B and most Medicare Advantage plans. The co-issuance and deductibles are waived by these plans so there is no out-of-pocket cost. MPAC Healthcare providers can now extend this valuable service as part of a patient's short-term rehab or long-term stay at your facility.
