Allan is an Adult-Gerontology Nurse Practitioner who obtained is Master's Degree from Resurrection University. He started out as a patient-care technician and after obtaining his RN license, he worked in various settings such as skilled nursing and assisted living facilities, medical-surgical, telemetry, and oncology hospital units. He is certified by both the American Nurses Credentialing Center and by the American Association of Nurse Practitioners. He also holds a Bachelor's degree in Biology from the University of Illinois - Chicago.

Amber has been in healthcare for the past 20 years. She began as an LPN in 1999. Amber quickly found passion in caring for patients in long-term care. She began her graduate studies while employed as a Director of Nursing in a small rural home. Amber graduated from Chamberlain College of Nursing with her BSN in 2017 and her MSN in 2019. She looks forward to continuing to provide care to the aging population.

Amy is a Licensed Clinical Social Worker who obtained her M.S.S.A at Case Western Reserve University while also earning the Graduate Certificate in Gerontology. She obtained her Bachelor of Arts in Social Work from Concordia University. Amy has worked in the Social Service field for over 10 years. She has worked in a variety of setting such as post-acute rehabilitation, custodial care, and hospice. Within these settings, Amy provided counseling and psychosocial support to Patients and their families as they work through transitions of care, housing concerns, family dynamics, and coping with their disease processes. Amy is very excited to learn and grow with the MPAC team.

Carmen is a Licensed Clinical Social Worker. She received her B.A. in Social Work from Northeastern Illinois University and earned both a Master of Social Work (MSW) with a concentration in aging practice and Ph.D. in Social Work from the University of Illinois at Chicago (UIC). Carmen’s career spans work with individuals and families as a direct practitioner, educator, and administrator. Her areas of interest/experience include social work practice with the elderly, especially Latino elderly; medical social work (general, rehabilitation, geriatric, inpatient, and home health); and mental health counseling. As an educator, she has taught across the curriculum and was the coordinator of field education for both MSW and BSW programs. Throughout her professional career, she has striven to raise organizational consciousness relative to the delivery of linguistically and culturally congruent services to vulnerable populations. She is a fluent native Spanish speaker.

Diane is a caring and compassionate provider. She enjoys taking care of her patients and their families with a holistic approach. She joins MPAC with 10 + years of nursing experience. She graduated from Bradley University with a BSN and started her nursing career as an ICU nurse at Alexian Brothers Medical Center - during which she was a staff nurse, charge nurse, mentor, and member of the rapid response team. As she looked to grow and expand her nursing career she completed her MSN-FNP in Dec 2009 from Northern IL University. Working as an APN for the last several years she has had the opportunity to provide care for a variety of patients in the areas of wound care, post-acute care and primary care - delivering quality patient care as well as being an advocate for illness prevention through patient and family education. Diane enjoys working with the geriatric population and looks forward to learning new things and working as a team member with colleagues.

Marcy is a Licensed Clinical Social Worker, Certified Dementia Practitioner, and a member of the Academy of Certified Social Workers. She earned her bachelor's degree in Criminal Justice from Washington State University and her Masters of Social Work from Aurora University. Her experience includes nephrology social work, emergency room crisis triage, mental health assessments, and referral, group, and individual counseling for individuals with intellectual and developmental disabilities and dually diagnosed. Developing and implementing person-centered programming for individuals who are experiencing functional and cognitive decline. Marcy is excited to join MPAC and be apart of this strong clinical team!

Sara Yant is a Licensed Clinical Social Worker who graduated from the University of Illinois, and has worked in social services for over 20 years. She has worked in various capacities including Alzheimer's care, caregiver/family support, group work, and most recently, as a mental health therapist for individuals and families in the community.
Her approach to care includes utilizing and valuing an individual's life story (narrative), identifying strengths and assisting in steps to reach goals. She is passionate about coming alongside individuals and families navigating unique challenges and opportunities that arise with changes in aging, life transitions, and issues related to memory loss.
She is excited about joining the MPAC Healthcare Team that aims to reach and support an often overlooked population.
